Before filter behavior inconsistent

When using a block with a before filter, it doesn't execute for all the hosts it should. Using a task in the before filter does execute for all the hosts. See the attached capfile for an example.
This happens with cap 2.3 - I'm pretty sure it didn't happen in 2.1, never tried 2.2.
